Feedback and Complaints — Marching Forward
Feedback and complaints

Tell us what's working, and what isn't.

We welcome all feedback, including complaints and compliments. If you have a complaint or concern about your NDIS supports or the services Marching Forward provides, it's important to talk about it.

We respond within three business days

Once you send the form, a member of our team will be in touch.

Kept confidential

We will endeavour to keep the information in your complaint confidential, unless we are required by law to share it.

You can stay anonymous

You don't have to tell us who you are. See the two ways to do that below.

Anonymous complaints

Prefer not to give your name?

That's completely fine. You can make an anonymous complaint in either of these ways.

1

Call us

Phone 0447 427 142 and say you wish to make an anonymous complaint, so we don't ask you to identify yourself.

2

Leave your details off the form

Fill out the form without your name, phone or email, as these may identify you. You can also print the form and post it to 14 Roderick Street, Dayboro QLD 4521.

The form

Feedback and complaints form

Include as much detail as you can, such as the date and time of what happened and an outline of the issues involved, and tell us the outcome you would like.

Need help with communication, such as an interpreter or the National Relay Service? Tell us in the form or call 0447 427 142.

Another option

Complaining to the NDIS Commission

You can also make a complaint to the NDIS Quality and Safeguards Commission at any time. The Commission can take complaints from anyone about NDIS services or supports that were not provided in a safe and respectful way, or not delivered to an appropriate standard.

How to contact the Commission
