A structured, supported program helping teens build confidence, independence, and social capability through community engagement.

Community Connections is a 13-week, NDIS-aligned program designed for young people aged 12–17 yrs. The program focuses on developing social confidence, independence, emotional regulation, and real-world skills through safe, guided experiences in the community.

Each participant engages in a personalised pathway that reflects their strengths, interests, and NDIS goals, ensuring that every session is meaningful, growth-oriented, and outcome-driven.

BONUS: Activity Costs Covered!

Participants are only charged for support in line with their NDIS plan.

As a value-added inclusion, Marching Forward covers the cost of structured program activities (such as entry fees to theme parks, movies, and selected community experiences).

NDIS funding does not cover activity entry fees — however, we absorb these costs to ensure participants can fully engage in meaningful experiences without additional financial pressure.

How the Program Works

  • Small group support:
    Participants engage in a high staff-to-participant ratio (max 1:2) to ensure safety, personalised guidance, and meaningful skill development.

  • Community-based learning:
    Weekly sessions and excursions allow adults to practise real-world skills, strengthen peer networks, and build confidence navigating community spaces.

  • Goal-oriented and personalised:
    Each participant has an individual Success Plan aligned with their NDIS goals, guiding their development across social, emotional, and independence domains.

  • Capacity-building focus:
    Supports outcomes in independence, social participation, emotional regulation, decision-making, and practical life skills.

NDIS Alignment & Professional Integrity

Community Connections is a specialist NDIS program and can be funded through Core Supports or relevant Capacity Building supports depending on participant goals.

The program is delivered in alignment with NDIS Practice Standards and informed by strengths-based, neuro-affirming, adult-focused approaches.

Prior to commencing, each participant completes a comprehensive onboarding process to:

• Understand their strengths, interests, and support needs

• Identify and align program strategies with their NDIS goals

• Develop a personalised plan to guide their journey throughout the program

This Is an NDIS-Funded Program?

Community Connections is an NDIS-aligned, capacity-building program and may be funded through Core Supports or Capacity Building, depending on your goals and plan. NDIS funding covers support, not activity entry costs.

To remove barriers to participation, Marching Forward covers the cost of scheduled program activities — giving participants access to premium community experiences without additional charges beyond their approved supports.

Funding is typically accessed under:

• Assistance with Social, Economic and Community Participation – 04_102_0136_6_1

• Access Community, Social and Recreational Activities – 04_104_0125_6_1

• Increased Social and Community Participation – 09_009_0117_6_3

Invoices are issued weekly across the 13-week program.

·Note: Participants are required to bring lunch or money for lunch during community-based sessions.

Where is the program located?

Community Connections is delivered through community-based sessions and structured outings.

Our primary delivery location is in Dayboro, with excursions and activities occurring within the broader Brisbane, Sunshine Coast & Gold Coast areas.

If transport support is required, this can be discussed during onboarding to determine individual suitability.
